regulator question

on my rs it started leaking above the regulator. i tightened it but got a secondary leak by the on off valve. eventually i fixed it but are there any ideas why it started leaking.

lack of lube on an ORing, bad ORing, dirty ORing (CO2 vs HPA), over pressurization are generally the culprits
